In this online learning module, participants will learn about why it is important to have and document Goals of Care discussions in a systematic way in the primary care setting for patients with complex care management needs. They will learn about a team-based ThedaCare pilot initiative designed to identify patients at high-risk of misalignments in plans of care and facilitate Goals of Care discussions and appropriate continuing services and/or onward referrals. Participants will learn how such discussions can be documented and reviewed using a novel Goals of Care flowsheet in the electronic health record, as well as how to have appropriate, comprehensive, compassionate, and patient and family centered Goals of Care discussions.
