PANDAS and PANS: The Controversy

Pediatric Autoimmune Neuropsychiatric Disorder Associated with Streptococcus (PANDAS) and Pediatric Acute-Onset Neuropsychiatric Syndrome (PANS) are clinical syndromes occurring in children which are characterized by the abrupt onset of neurobehavioral symptoms (most often obsessive-compulsive disorder). PANS is the umbrella term for the disorder. When the instigating event is group A streptococcus, the syndrome is referred to as PANDAS. The biologic plausibility for these disorders is provided by the example of acute rheumatic fever which is precipitated by a throat infection with Group A streptococcus. This activity discusses acute rheumatic fever and Sydenham chorea as a model for PANDAS and PANS and reviews the diagnostic criteria and approach to management of these entities.
