This program is a collaborative effort with the UNM Center for Diabetes and Nutrition Education (UNM CDNE) and the Department of Health Diabetes and Prevention Control Program. The sessions serve as a community of practice for organizations and providers interested in achieving DSMES program accreditation, as well as for organizations that are working to maintain and improve their accredited programs. This ECHO program is designed to build from the content delivered in year one of the diabetes health equity grant, and equip participants with the knowledge and resources to effectively navigate the accreditation and re-accreditation processes and leverage the benefits. Utilizing the ECHO Model, the multidisciplinary team will mentor participants through didactic sessions and case-based learning, aimed at strengthening access to diabetes self-management education and support services within our communities. Goals of this program include supporting organizations to address DSMES program participation barriers by adapting and tailoring the content and approach for priority populations. We also aim to strengthen referral policies and practices, establish connections with complementary diabetes support programs, and increase awareness of DSMES services, with a focus on southern New Mexico.
