Systems change is an important part of complex care that helps resolve root issues of the barriers faced by people with complex health and social needs. It is a core part of the complex care frontline practitioner’s job both because of the access that the frontline practitioner has to stories and experiences navigating the systems but also because systems change work can be empowering and prevent burnout. This module teaches learners to identify a system issue, collect data about that issue, and then identify an audience and craft a story to help resolve the issue. Learners will be given the tools to actually implement systems change projects in their organization and community.
