Mavacamten is a novel, first-in-class oral medication approved by the US Food and Drug Administration (FDA) as an allosteric modulator of cardiac myosin ATPase to treat obstructive hypertrophic cardiomyopathy (HCM) in patients. This medication is currently indicated for treating adults with symptomatic heart failure classified as New York Heart Association (NYHA) class II and III secondary to obstructive HCM and is aimed to improve functional capacity and alleviate symptoms.HCM is a common heritable cardiac disorder marked by left ventricular hypertrophy due to mutations in genes related to cardiac sarcomeres, such as myosin-binding protein C and β-myosin heavy chain. Mavacamten works by targeting the underlying myofibrillar disarray and abnormal mitral apparatus that contribute to left ventricular outflow tract (LVOT) obstruction in HCM.
