Uveitis glaucoma hyphema (UGH) syndrome is a relatively rare clinical condition characterized by chronic postoperative inflammation, high intraocular pressure (IOP), and hemorrhage in the anterior chamber of an eye post-cataract surgery with intraocular lens (IOL) implantation. The condition is described in association with the closed-loop anterior chamber IOLs and concurrently in eyes with modern IOLs. The presence of uveitis, glaucoma, and hyphema characterizes the UGH syndrome. The inflammation associated with uveitis may directly affect the drainage pathways in the eye, creating a barrier to the outflow of aqueous fluid; this causes the IOP to rise, leading to features of glaucoma. Hyphema complicates matters since blood in the anterior chamber may obstruct outflow channels and exacerbate inflammation. The cause of inflammation is usually the IOL irritating the iris and other intraocular structures.This activity reviews the diagnosis, cause, and mechanisms of the development of UGH syndrome and provides a detailed review of required medical and surgical management. Participating clinicians will learn how to work within an interdisciplinary team to improve patient’s quality of life by preserving optimal vision.
