A parahiatal hernia is a rare type of diaphragmatic hernia where abdominal contents herniate through a defect in the diaphragm that is not the usual hiatus. The presentation depends on the extent of bowel herniation, ranging from asymptomatic to life-threatening if the bowels are strangulated, obstructed, or perforated. These hernias are associated with increased morbidity and mortality if not identified and managed appropriately. The rarity of this condition could be due to one or a combination of the following factors: confusion about the anatomy, similarity in clinical presentation with paraesophageal hernias, and infrequent reporting in the literature.Imaging can aid in the diagnosis and evaluation of the condition’s severity. The management approach relies on the severity of the symptoms, with conservative measures sufficing in uncomplicated cases and surgery warranted in the presence of severe gastrointestinal pathology.This activity for healthcare professionals is designed to enhance learners’ competence in evaluating and managing parahiatal hernia.
