Cirrhotic cardiomyopathy (CCM) is a cardiac dysfunction occurring in patients with cirrhosis, manifesting without other known heart diseases. CCM affects up to 50% of all cirrhotic patients and typically manifests under stress. This condition is characterized by both systolic and diastolic dysfunction, along with structural heart changes, such as chamber enlargement, and a hyperdynamic circulatory state. CCM presents with impaired cardiac response to stress, a hyperdynamic circulatory state, and various electrophysiological abnormalities, notably QT prolongation. Due to its stress-induced nature and the lack of standardized treatment, CCM often leads to misdiagnosis.The diagnostic criteria established at the 2005 World Congress of Gastroenterology require evidence of systolic and diastolic dysfunction, alongside supportive criteria such as QT prolongation and elevated cardiac biomarkers. Diagnosis is confirmed through laboratory and imaging studies, including echocardiography, often supplemented by stress tests to demonstrate impaired cardiac responses. Currently, treatment follows standard heart failure management protocols; however, liver transplantation remains the cornerstone of CCM therapy, significantly improving cardiac function after surgery.
