Middle Ear Cholesteatoma

Cholesteatomas are benign but expansive aggregations of keratinized squamous debris within the middle ear, which can damage surrounding structures. Cholesteatomas are typically unilateral and can lead to hearing loss, dizziness, and facial muscle paralysis if left untreated. These slow-growing and insidious lesions usually require surgical removal and meticulous medical care. Middle ear cholesteatomas can be classified into 3 major types—congenital, primary-acquired, and secondary-acquired. Congenital cholesteatomas are rare and can arise from squamous epithelium trapped within the temporal bone during development. Primary-acquired cholesteatomas result from pressure changes in the middle ear that cause tympanic membrane retraction, leading to potential damage to the ossicles and erosion of the tegmen mastoideum. Secondary-acquired cholesteatomas develop due to direct injury to the tympanic membrane, usually from infection or trauma.The diagnosis is clinical and supported by history, physical examination, audiometry, and imaging. Treatment is primarily surgical, aiming to eliminate the disease, control infection, and preserve or restore hearing. Long-term monitoring and management are crucial due to the risk of recurrence and potential complications. This activity reviews the pathophysiology, clinical presentation, postoperative complications, long-term monitoring, management of recurrent disease, and treatment of patients with primary- and secondary-acquired cholesteatomas. This activity highlights various nonsurgical and surgical treatment approaches, including postoperative care and follow-up, while emphasizing the collaborative roles of the interprofessional healthcare team. In addition, this activity also enhances the diagnosis, treatment, and management of middle ear cholesteatoma, ultimately improving patient care and quality of life.
