This online learning module is intended for a multispecialty audience. Early recognition of dementia can help reduce the emotional, physical, and financial impact on those with dementia and their caregivers. Healthcare providers across the continuum of care may pick up on clues that a further discussion or referral is needed.

This module will provide basic information on Alzheimer’s Disease and related dementias. Additionally, this module offers guidance on using the Chronic and Principle Care Management billing codes to support care coordination for patients with dementia and their caregivers. The Alzheimer’s Association provides many resources for clinicians, patients, and caregivers for the breadth of dementia diagnosis, not just Alzheimer’s Disease.
