On the Frontline for Dementia Care

Dementia is highly prevalent in aging and has significant psychosocial and economic impact. Despite its prevalence, dementia often is under detected and underdiagnosed, sometimes even years after a patient has onset of symptoms. According to the 2016 VHA Dementia Steering Committee recommendations, most of the dementia diagnosis and care management should occur within the Patient Aligned Care Teams (PACTs). Veterans have a greater risk of experiencing risk factors that increase the possibility of being diagnosed with dementia. PACTs may be unfamiliar with understanding dementia, early detection, assessment and coordination of care. This information is essential to healthcare teams as they stand on the frontlines and play an integral role in providing care for these patients and their care partners. This recorded virtual, knowledge-based webinar will inform Patient Aligned Care Teams about dementia and how they may best detect, differentially diagnose, and provide care to patients and their care partners. As a result PACT clinicians will have the knowledge to recognize warning signs of dementia, and in response, be able to conduct a diagnostic evaluation, as well as develop and implement a Veteran-centered treatment plan.
