Medication errors have been a highlighted safety issue targeted by The Joint Commission (TJC) due to an estimated 7 million patients being affected annually at a cost of $21 billion (Da Silva & Krishnamurthy, 2016; Donaldson et al., 2000). Further, medication errors cause 7,000 U.S. deaths annually (Da Silva & Krishnamurthy, 2016; Donaldson et al., 2000). Medication errors occur for multiple reasons and are usually not due to the faulty judgment of a single person. Instead, they often indicate a system that is flawed and cannot adequately detect mistakes.
