Obstetrical hemorrhage is one of three leading causes of maternal morbidity and mortality worldwide. Postpartum bleeding is the most frequent cause of obstetrical hemorrhage and a substantial contributor to maternal mortality, accounting for approximately 11% of maternal deaths in the United States. It also remains a significant cause of maternal morbidity for any woman experiencing blood transfusion. More than half of the maternal deaths occur within 24 hours of delivery. Post-delivery bleeding presents risk for the entire obstetrical population and is the cause of approximately 700 maternal deaths in the U.S. each year. It is now known that >90% of postpartum hemorrhage (PPH) may be preventable. Lack of preparedness and medical error can lead to significant morbidity and mortality. Hospitals and OB providers/team members need to recognize the serious threat of hemorrhage and be well prepared to handle the emergency of post-delivery bleeding.
In this course, we inform OB providers/team members about: PPH definitions, etiologies, readiness, recognition and risk assessment, stage-based response, and associated risk/safety/liability. We present and discuss case examples throughout. After completing this course, OB providers/team members should be able to: define primary and secondary PPH; recognize the pathophysiology of mechanisms for control of postpartum bleeding and the various etiologies of PPH; perform risk assessment for hemorrhage antepartum, intrapartum and postpartum; perform quantification of blood loss with a standardized methodology; and identify role-specific responsibilities for each stage of PPH.
