Pseudologia fantastica (PF), also known as pathological lying or mythomania, is a mental disorder characterized by persistent, pervasive, and often compulsive lying. PF causes dysfunction in many realms of life. Individuals with PF create elaborate, exaggerated narratives that they believe to be true, distinguishing this behavior from typical lying, which involves deliberate deception for a specific purpose. Despite its long history and significant impact, PF is not recognized as a distinct entity within diagnostic systems. The diagnosis of PF involves psychological testing, comprehensive medical evaluations, and gathering collateral information. Treatment typically includes cognitive behavioral therapy and medications, such as serotonin reuptake inhibitors and serotonin-norepinephrine reuptake inhibitors, to address comorbid conditions such as depression or anxiety.Management is complex and requires a multidisciplinary approach. Forensic implications of PF include challenges in assessing the competency of individuals to stand trial and the reliability of their testimony. Despite its prevalence and impact, research on PF is limited, underscoring the need for further study to improve diagnostic and treatment strategies. This activity reviews the current understanding and complexities of PF, including its epidemiology, the characteristics of people who lie excessively, current research, and the assessment, diagnosis, and treatment of the disorder to deliver appropriate care to affected individuals.
