An aortico-left ventricular tunnel is a rare congenital cardiac malformation characterized by an abnormal endothelial channel between the ascending aorta and the left ventricle. This extracardiac passage allows blood to flow directly from the aorta to the left ventricle, bypassing the normal route through the aortic valve. This condition often leads to heart failure if left untreated, as the continuous abnormal flow increases the volume load on the left ventricle. There are four anatomic types of these tunnels, which can be differentiated based on their origin and course. The tunnel typically originates just above the sinotubular junction near the right coronary sinus, with the ventricular opening located below the aortic valve in the interleaflet triangle between the left and right leaflets.
