Mohs micrographic surgery (MMS) is a highly effective, gold-standard treatment for managing many cutaneous malignancies due to its high cure rates and tissue-conserving nature. In MMS, unlike other surgical techniques, the surgeon also serves as the pathologist, ensuring complete histological clearance of the tumor. Specialized processing techniques are used to prepare, section, and stain tissue, allowing thorough examination of both peripheral and deep margins. The tissue is prepared using a cryostat to create frozen sections, which are stained with hematoxylin and eosin stains or immunostains, such as MART-1 for melanoma.Proper specimen processing, including the use of a cryostat and various staining methods (such as immunostaining), is crucial for the accurate interpretation of fresh-frozen sections and successful MMS outcomes. High-quality slides allow the surgeon to thoroughly evaluate tumor margins and characteristics, minimize complications, and achieve high cure rates. As tissue conservation is a key aspect of MMS, well-prepared specimens help prevent false-positive margins and processing artifacts, avoiding unnecessary removal of additional skin and reducing patient harm. This activity reviews techniques for preparing and sectioning specimens using a cryostat, staining, and immunostaining, providing surgeons with the essential knowledge to properly process tissue, identify and troubleshoot common processing errors, and enhance patient care.
