While advances in antiretroviral therapy have made it easier than ever to treat HIV, it remains challenging to ensure that people living with HIV receive the holistic medical and support services they need to thrive. Models for coordinated care delivery, such as the patient-centered medical home, can improve patient outcomes and engagement. However, implementation of these models must be carefully tailored to the specific needs of community settings where critical services are often fragmented across multiple locations, specialists, and community-based organizations.
