This one-hour, on-demand, journal article-based activity presents original research on models that support successful transitions to long-term medical and substance use disorder (SUD) care for hospitalized patients with SUD. Using an expert consensus approach, existing SUD care transition models were evaluated for effectiveness and implementation with an analysis of factors associated with the greatest potential to enhance posthospitalization care transitions. According to the expert panelists, the models considered to have the greatest potential to enhance posthospitalization SUD care transitions were the interdisciplinary addiction consult team (ACT), in-reach from partnering outpatient clinics, and bridge clinics.
