Case-based learning: PCI in Post-TAVR Patients

Patients with prior transcatheter aortic valve replacement (TAVR) may require coronary angiography and percutaneous coronary interventions (PCI). Operators performing TAVR routinely are familiar with different transcatheter heart valve (THV) platforms and their features, as it relates to coronary access. However, operators not performing TAVR may not be fully aware of the differences in the various THV platforms and the implications for coronary access. This is important because non TAVR operators often take care of patients with prior TAVR and perform coronary angiography and PCI either electively or urgently in the setting of acute coronary syndromes. The purpose of this activity is to familiarize operators with the different THV platforms commercially used in the United States. The first part of the activity is drag and drop, to identify the different THV platforms. This is followed by a discussion of the salient features of the Sapien, Evolut and Navitor THV platforms. We then discuss each THV platform individually, specifically focusing on the valve design, size, dimensions and the challenges when it comes to coronary access and performing PCI in a patient with that THV. Commissural misalignment can make coronary access challenging, particularly with a tall frame valve such as Evolut. Using interventional techniques such as guide catheters, air mailing guidewire technique and guide extension catheters leads to success with coronary access and PCI in patients with challenging access post TAVR.
