Preventable hospital readmissions are a significant avoidable expense in the U.S. healthcare system costing billions of dollars annually, while needlessly placing quality healthcare at risk. Common causes of readmissions include unsatisfactory communications among providers and between providers and patients, insufficient transitional policies and processes as patients move through the continuum of hospital care (admissions through post-discharge), and a lack of proactive accountability in anticipating and addressing potential adverse events. As the cost of medical care continues to rise, government legislators and healthcare administrators alike are seeking ways to reduce costs. Avoiding unplanned and unnecessary hospital readmissions is one area that has the potential for cost savings. However, our current healthcare system often falls short of delivering simple, systematic and process-driven strategies to effectively address and resolve this dilemma.
In this activity, we focus on root causes and viable solutions; we introduce seven strategies that clinical providers can easily implement to reduce hospital readmissions, decrease unnecessary expense in the U.S. healthcare system, and greatly improve patient care. After completing this activity, practitioners should be able to: assess the importance of employing strategies to decrease hospital readmissions; identify patients at high risk for readmission within 30 days after hospital discharge; and employ strategies to reduce the risk of preventable re-hospitalizations.
