Apocrine hidrocystoma is a rare, benign cystic tumor originating from the secretory part of the apocrine sweat glands. The tumor typically presents as a solitary, asymptomatic papule or nodule and is commonly found on the head and neck, although it may occasionally appear in other areas. The exact cause and pathogenesis of this condition remain unknown. Currently, apocrine hidrocystoma is regarded as a cystic proliferation of apocrine glands rather than a simple retention cyst.Although apocrine hidrocystoma is often a solitary growth, multiple apocrine hidrocystomas can also occur in 2 rare ectodermal dysplasias—a specific form of Goltz-Gorlin syndrome and Schopf-Schultz-Passarge syndrome. In addition, a unique presentation combining multiple apocrine hidrocystomas with additional clinical findings, including bilateral keratoconus, esophageal papillomatosis, and hiatus hernia, has been documented in a particular form. Histological examination typically confirms the diagnosis, while dermoscopic findings may reveal various patterns and structures. Given the benign nature of the lesion, surgical excision with narrow margins is the primary treatment. This activity outlines the epidemiology, genetics, and clinical presentation of apocrine hidrocystoma, emphasizing its rarity, diagnostic approaches, and management strategies. This activity provides clinicians with the necessary knowledge and tools to diagnose and treat this relatively common yet complex condition and enhance patient care and outcomes.
