Barrett Esophagus

Barrett esophagus is a premalignant condition partly related to esophageal reflux, characterized by gastric and, more importantly, intestinal columnar metaplasia of the normally squamous mucosa of the distal esophagus. The columnar epithelium in the affected portion of the esophagus may become dysplastic and subsequently progress to esophageal adenocarcinoma or adenocarcinoma of the esophagogastric junction. This progression occurs in steps, therefore, mucosal biopsies of the esophagus may not show dysplasia, or show indefinite, low- or high-grade dysplasia, or cancer. Because this progression occurs over many years, there are opportunities to screen for Barrett mucosa in high-risk individuals, to monitor the condition periodically over several years after Barrett mucosa is identified, and to treat the pathology in various ways at each step of the progression. Intensified medical therapy to reduce acid reflux is the first step in managing Barrett esophagus, whether or not symptomatic reflux is being treated adequately. While esophagectomy was the only treatment option for high-grade dysplasia and cancer in past years, there are now various endoscopic options for resection or destruction of the affected tissue.This activity illustrates the pathophysiology, risk factors, diagnostic criteria, treatment, and complications of Barrett esophagus—including the importance of histological assessment, the role of endoscopic surveillance, and the integration of novel therapies into clinical practice. The activity also highlights the role of an interprofessional healthcare team in optimizing treatment strategies and enhancing patient outcomes in clinical practice.
