Bronchopleural fistula (BPF) is a severe postoperative complication in thoracic surgery, characterized by a sinus tract between the bronchus (main stem, lobar, or segmental) and the pleural space. Causes include surgical complications, pulmonary infections causing necrosis, spontaneous pneumothorax, chemotherapy, radiotherapy, and tuberculosis. Patients with BPF present with a range of symptoms, from acute tension pneumothorax to subacute empyema, typically within the first 2 weeks following lung resection. Diagnosis involves clinical assessment, blood tests, chest imaging, and flexible bronchoscopy. Treatment strategies vary from medical management and bronchoscopic procedures to surgical interventions. Monitoring after closure involves clinical assessment, chest tube output, and imaging to detect recurrence or complications. Persistent or complex cases might necessitate repeat interventions or, rarely, open-window thoracostomies. This course enhances clinicians’ competence in recognizing and managing BPF through a comprehensive understanding of its pathophysiology, diagnostic methods, and treatment options. Participants learn to employ a multidisciplinary approach, collaborating with specialists in surgery, pulmonology, and infectious diseases to optimize patient care. Clinicians improve diagnostic accuracy and treatment outcomes by emphasizing timely intervention and individualized treatment strategies, bridging practice gaps associated with BPF management.
