Complex Care Course 1: Introduction to complex health and social needs

Complex care is a person-centered approach operating at the individual, community, and system levels to improve health and well-being for people with complex, multiple, and intersecting medical, behavioral health, and social needs. This includes individuals with multiple chronic physical and behavioral health conditions who face social barriers, such as homelessness, unstable housing, food insecurity, or lack of transportation, that often result in cyclical patterns of unmet healthcare and social needs. Both the chronic health conditions and social barriers are often, but not always, exacerbated by systemic inequities such as racism, ableism, ageism, and poverty. Complex care seeks to promote health equity by helping people with complex needs identify and meet their own health and well-being goals through coordinating and strengthening access to services and supports while building self-efficacy. This course will introduce the learner to the varied and interconnected needs of individuals who engage with complex care.
