Congenital Hypertrophy of Retinal Pigment Epithelium

Congenital hypertrophy of the retinal pigmented epithelium (CHRPE) is a benign ocular condition characterized by well-defined, flat, pigmented lesions primarily located in the fundus of the eye. CHRPE lesions are typically unilateral and solitary, although multiple or bilateral occurrences are possible, and they are often asymptomatic, usually discovered incidentally during routine eye examinations. While generally benign, CHRPE lesions may exhibit subtle changes over time, such as slight enlargement or alterations in pigmentation. Although CHRPE lesions typically do not cause visual disturbances, further evaluation may be warranted to rule out related syndromes or differential diagnoses, especially in cases of multiple or atypical lesions. Management involves periodic monitoring to detect changes in size or morphology, with intervention reserved for cases with concerning features or associated pathology. A thorough systemic evaluation is recommended in cases of atypical or multiple CHRPEs to exclude related syndromes such as familial adenomatous polyposis. Treatment is generally unnecessary for solitary CHRPE and grouped retina pigmentation, with observation being the primary approach. In cases associated with syndromes such as familial adenomatous polyposis, collaboration with other specialists may be necessary, and regular follow-up and patient education are crucial. In addition, routine endoscopies and genetic screening are recommended in some cases. This activity aims to enhance the competence of healthcare professionals in identifying CHRPE, conducting the recommended evaluation, and implementing an appropriate interprofessional approach for managing this condition.
