This training module introduces participants to Age-Friendly Health Systems. Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute of Healthcare Improvement in partnership with the American Hospital Association and the Catholic Health Association of the United States.
The four components of the system are What Matters, Medication, Mentation, and Mobility. Optimal implementation of Age-Friendly Health Systems requires an inter professional team effort.
