Six years. Eleven states. Twenty CDC-recognized programs across 41 counties. More than 80 ADCES lifestyle coaches working within inter-professional care teams to reach 5000 people with prediabetes. CDC’s 1705 Cooperative Agreement provided a historic opportunity to explore what works to engage and retain participants in the National Diabetes Prevention Program, advance health equity for priority populations, and provide an integrated approach to sustain prevention long-term.
