Ectopic Pregnancy #1: Diagnostic Error & Risk Reduction

The risk of missing an ectopic pregnancy is significant. Rupture of an ectopic pregnancy can result in life-threatening hemorrhage. Even with diagnostic and technological advancements, ectopic pregnancy is missed between 2% and 8% of the time at the first office visit and between 4% and 17% of the time at the first emergency department visit. In this activity, we present and discuss actual cases to examine some of the common causes of medical errors related to the evaluation and diagnosis of ectopic pregnancy. After completing this activity, providers/members of the healthcare team should be able to: maintain a high index of suspicion for ectopic pregnancy at the time of the first visit for all women of childbearing age with symptoms that may be related to pregnancy, whether ectopic or otherwise; evaluate women of childbearing age presenting with a chief complaint of lower abdominal pain or vaginal bleeding with an awareness of the common causes of the failure to diagnose ectopic pregnancy; create a standard approach to the evaluation of women of childbearing age presenting with a chief complaint of lower abdominal pain or vaginal bleeding that includes an analysis of risk factors for ectopic pregnancy; and apply strategies for preventing similar incidents of failure to diagnose ectopic pregnancy, including a team-based approach and effective communication between healthcare providers.
