Enhancing Partnerships to Address Birth Defects and Infant Disorders, and Related Conditions (VLC)

The AAP is engaged in the 5-year CDC-funded grant Improving Clinical and Public Health Outcomes for Infants and Children with Birth Defects and Infant Disorders (BDID) and Their Families in Disproportionately Affected Communities, and one of the program objectives is to strengthen discharge education and best practices among hospital and community-based providers to improve transition from hospital to home for infants with BDID. The AAP will be planning and facilitating QI virtual learning collaboratives with cohorts of up to 9 hospitals each year of the project. The QI effort will provide physicians and non-physician clinicians with evidence-based best practice, strategies for implementation, and tools to bring about sustainable changes while also assessing the implementation strategies associated with the greatest success. Project Aim(s): Global Aim: To improve the discharge outcomes for infants with BDID, especially those with cCMV, gastroschisis, and CS in disproportionately affected communities. Specific Aim #1: Enhance the competence and expertise of healthcare professionals, both physicians and non-physician clinicians, to mitigate disparities and improve accessibility to integrated services for families affected by BDID. Specific Aim #2: Create an online toolkit on cCMV, gastroschisis and CS to disseminate amongst members (specifically Neos, Newborn Hospitalists and Outpatient Peds. Specific Aim #3: To improve outpatient referrals for infants with BDID to the appropriate subspecialists and communicate with the subspecialists and Pediatricians prior to discharge by 75%.
