Currently health care providers may not have adequate knowledge and/or skills to appropriately manage common geriatric conditions and syndromes. The top geriatric education needs requested on the recent Learner Needs Assessment include mental health, Alzheimer s/dementia, legal issues (guardianship, power of attorney, advance directives), chronic pain, depression, and the management of chronic conditions. The purpose of this recorded knowledge-based session is to address the challenges with prevalence of Potentially Inappropriate Medication (PIM), use and discuss innovative, evidence and data driven approaches in older population utilizing Vital, Important, Optional, Not indicated, and Every medication has an indication (VIONE) methodology. 8+ years of cumulative national and international experiences implementing VIONE will be discussed. Presenters will discuss effective partnerships, informatics, formal and informal implementation strategies for effective paradigm shifts to conceptualize and apply deprescribing practices in care of older adults to reduce harm and improve patient outcomes. Updates to the implementation of VIONE in VA will be presented with instructions on accessing and utilizing the tool in Computerized Patient Record System (CPRS) and using the VIONE implementation dashboard.
