Communication failures have been identified as the most important cause of preventable patient harm by The Institute of Medicine/National Academy of Medicine and The Joint Commission (TJC). They are also the major cause of medical malpractice claims. The most error-prone type of communication is the handoff, deemed responsible for 35% of all sentinel events. Handoffs are often on-the-fly conversations between practitioners without any designated format, a non-method that results in inadequate information transfer. The 2018 HATRICC study of OR-to-ICU handoffs has validated the use of a standardized format for reporting off, and method of promoting the cultural changes necessary to make the handoff a team effort and a patient-centered process.
