Healthcare Provider Impairment: Identification & Management

There are a number of terms that describe impairment in physicians. Impairment is a functional term. A person may be ill, but not impaired. The term applies to impairment due to physical illness, disability, mental disorder, or substance use disorders, including abuse and dependency on drugs and alcohol. Impairment in healthcare providers is generally the inability to practice medicine with reasonable skill and safety. This course will focus on impairment due to the use of illicit drugs or alcohol. The rate of substance use disorder in healthcare providers is not markedly different than the rate in the general population. While substance use disorders are a public health problem across the board, what is different with healthcare providers is the effect that impairment can have on patient safety. It is difficult enough to practice medicine safely and effectively without any impairment due to substance use disorders. Attempting to practice medicine while impaired can lead to catastrophe.

In this activity, we will discuss identifying the impaired clinician, how to conduct an intervention, treatment options, and returning to work after treatment. After completing this activity, practitioners should be able to: identify the signs of impairment in another healthcare provider; describe interventions commonly necessary for confronting healthcare providers about their substance use issues; and describe the issues surrounding the impaired healthcare provider’s return to practice.
