Level III Heart Failure Focused 2.0 directs the healthcare team through the collaborative process of transitioning the heart failure patient to the next appropriate level of care. The importance of identifying, coordinating, and communicating the patient s individual needs for consults, referrals, and other considerations are addressed to ensure seamless care transitioning. We promote success by providing heart failure instructional education, including healthy lifestyle and self-care management strategies, to effectively teach the team to teach. Lastly, we address the components and benefits of heart failure centers of excellence and the quality and performance measures that help to achieve program excellence.
