This webinar will explore the definition of team-based care and provide participants with the opportunity to identify who, what, why, how, and when they can implement strategies to enhance their team-based care approach to caring for their youngest patients. Participants will learn who are the essential members of any team (including the family), learn specific models and strategies that facilitate team-based care and, hear case examples of effective team-based care implementation that will illustrate what this could look like in their practice. Action plans for next steps will be discussed and attendees will be encouraged to identify site-specific strategies they can implement in the short- and long-term to enhance their team-based care approach.
