The goal of this activity is to improve the rates of body mass index (BMI) documentation for patients aged 18 years and older who were seen at least twice for any visits or who had at least one preventive care visit during the two-year measurement period. In support of physicians board certification, this Performance Improvement – Continuing Medical Education (PI-CME) activity is designed to support medical practices quality improvement (QI) initiatives for the purpose of earning CME credits and MOC Part IV points. During this activity, the learner will actively work towards improving their practice by assessing their current practice, learning best practices and guidelines relevant to the content, implementing changes to their practice, and evaluating and reflecting on the changes they made to their practice. Each activity is divided into separate assessment stages: 1. Baseline measurement and intervention planning stage – 5 AMA PRA Category I Credits awarded upon completion. 2. Interventions implementation stage. At least 7 days of intervention implementation is required – 5 AMA PRA Category I Credits awarded upon completion. 3. Evaluation measurement and reflections – 5 AMA PRA Category I Credits awarded upon completion. 4. Completion and submission of final quiz – 5 AMA PRA Category I Credits awarded upon completion. Learners will earn a separate CME certificates for each stage completed.
