Millard-Gubler Syndrome (MGS) is a classical crossed-brainstem syndrome characterized by a unilateral lesion in the basis pontis (ventral pons). This neurological condition manifests with an ipsilateral lower motor neuron palsy of the facial nerve (CN VII) and a contralateral hemiparesis, attributed to the involvement of the corticospinal tract in the pons above the medullary decussation of the pyramids. Although the abducens nerve (CN VI) remains classically unaffected, some authoritative sources include an ipsilateral sixth nerve palsy as part of the clinical syndrome; CN VI involvement implies medial extension of the causative lesion. Diagnosis is suggested by clinical examination findings that reveal ipsilateral peripheral CN VII paresis (potentially also CN VI) alongside contralateral hemiparesis in the upper and lower extremities. MGS in younger individuals often results from infection or neoplastic lesions. In older adults, hemorrhage and infarction are more common antecedents of MGS.This review covers the etiology, clinical presentation, pathophysiology, differential diagnosis, evaluation, and management of Millard-Gubler Syndrome, emphasizing the crucial role of an interprofessional healthcare team in understanding and effectively managing affected patients.
