The modified Blalock-Taussig-Thomas (mBTT) shunt represents an advancement over the classical Blalock-Taussig (BT) shunt introduced in 1944. The mBTT is utilized to palliate specific cyanotic congenital heart lesions as a systemic-to-pulmonary shunt. Although its overall use has declined, the mBTT shunt continues to have a significant role in treating neonates with univentricular cardiac lesions. This procedure is often utilized in patients with severe pulmonary stenosis or atresia.Contraindications include pulmonary hypertension and significant left-to-right shunts, as the shunt may exacerbate these conditions. The prognosis following the shunt is generally favorable, providing temporary relief from hypoxia. However, long-term outcomes depend on the timing of subsequent corrective surgery. While the procedure carries risks such as shunt stenosis, thrombosis, and pulmonary overcirculation, it can lead to positive outcomes when followed by timely corrective intervention. This activity provides clinicians with a comprehensive review of the mBTT shunt, including its history, indications, risks, benefits, contraindications, surgical techniques, and common postoperative complications. This activity assists clinicians in identifying patients with indications for the mBTT shunt and performing its proper placement. In addition, this activity fosters effective collaboration within an interprofessional healthcare team caring for neonates with congenital heart disease undergoing this palliative surgical procedure.
