Unlike singleton pregnancies, the mode and timing of delivery for multifetal gestation is based on the type of twins or higher order multiples, with an evaluation of maternal and fetal complications. There are no clear guidelines on the best mode of delivery for all types of multiples. Techniques for vaginal delivery of twins include internal podalic version as well as breech extraction of twin B. Multifetal gestations, regardless of the mode of delivery, are associated with increased maternal and neonatal morbidity. To avoid the high morbidity associated with multifetal gestation, a multidisciplinary team must be available to provide adequate care to both the mother and her infants. This activity reviews multiple birth delivery and highlights the role of the healthcare team in managing and improving care for patients with multifetal gestation who undergo either vaginal or cesarean delivery. Clinicians can expect to gain a comprehensive understanding of the complexities and challenges associated with delivering twins or higher-order multiples.
