Necrolytic acral erythema is a rare dermatological condition primarily associated with chronic hepatitis C virus infection. This condition manifests as painful, erythematous, eroded, and occasionally bullous plaques, predominantly affecting the acral regions such as the hands and feet. The exact pathogenesis remains unclear, but it is believed to involve complex interactions between nutritional deficiencies, particularly zinc, and chronic liver disease. Necrolytic acral erythema is a significant cutaneous marker for hepatitis C virus, often appearing in patients with long-standing or poorly managed infections. The lesions typically present in 3 stages—initial, well-developed, and late, with characteristics ranging from small erythematous macules to thick, dark plaques with crusting.Diagnosing necrolytic acral erythema necessitates a thorough evaluation to identify underlying causes, especially hepatitis C virus infection, which is identified in more than 75% of cases. Although no specific laboratory tests exist, biochemical evaluations, including liver function tests and serum zinc levels, are essential.
