Neurobiology of Addiction

Historically, addictions were considered a manifestation of an individual’s ethical and moral shortcomings. Advances in neuroscience have fundamentally changed our understanding of addictive disorders. Currently, addiction is understood to be a chronic and relapsing disorder marked by specific neuroadaptations predisposing an individual to pursue substances irrespective of potential consequences. The neuroadaptations lead to a repetitive cycle comprising distinct stages: the intoxication/binge stage, the withdrawal/negative affect stage, and the preoccupation/anticipation stage. Activation of specific brain regions with subsequent neurotransmitter modulation distinguishes each stage in the cycle. Specific genetic, epigenetic, and molecular mechanisms may predispose an individual to the addiction cycle.This educational activity reviews the current understanding of the neurobiological stages of addiction, elucidates the role of nature and nurture in the addiction cycle, explores the impact of neurobiology on treatment modalities, and emphasizes the pivotal role of interdisciplinary teams comprehending these stages when delivering care to individuals with substance use disorders.
