New Mexico CHW Integration ECHO Program

In support of the establishment of an ECHO Hub team of subject matter experts to deliver didactics and guide discussion about the importance of, and process for, integrating CHWs and/or other Support Workers into healthcare organizations, UNM OCH has developed the New Mexico SDOH Support Integration curriculum for training health care leaders on SDOH and highlighting inclusion of CHWs and/or other Support Workers for providing social needs support across the spectrum. This SDOH expertise, developed by the UNM Office for Community Health, is leading to the creation of Health Extension Hubs in each county consisting of local employees as Health Extension agents (who develop SDOH resources where none exists), CHWs and other Support Workers (who assist individuals and families in navigation to needed and existing SDOH resources), and local learners (of all ages, interested in serving the health and social needs of their communities). These county-based hubs serve as on-the-ground resources for local hospitals, health systems, high schools and higher education, and local community-based organizations all of which can benefit not only by the knowledge in this curriculum, but also with knowledge of how to immediately apply this information to improve health in the local setting. This curriculum is an adaptation of the highly successful SDOH & Health Equity 101 Certificate Course, which is being deployed in all Extension Hubs and is part of the UNM Strategic Plan for Health Equity and CMS requirement for SDOH screening and follow-up of adult inpatients at all hospitals
