Food insecurity and nutritional inequity disproportionately impact underserved communities. These disparities are directly linked to higher rates of chronic diseases, such as obesity, diabetes, and cardiovascular diseases. Research shows that access to healthy food is limited in food deserts, exacerbating the health disparities experienced by these populations. Addressing these social drivers of health is essential to reducing the burden of chronic diseases and improving long-term health outcomes. In this CME Outfitters Snack, expert faculty will provide evidence-based strategies for healthcare professionals to address food insecurity by promoting community-based interventions, leveraging local partnerships, and integrating culturally tailored programs. Participants will explore the role of nutrition in managing chronic diseases and the importance of food access in preventive care, with the aim of reducing health disparities in underserved populations.
