Food insecurity and other negative social drivers of health (SDOH) impact health outcomes in underserved Black communities. Screening for food insecurity using validated tools has proven to be an essential part of healthcare interventions to connect patients to critical resources, such as SNAP, WIC, and local food banks. Research shows that food insecurity is strongly linked to chronic conditions like diabetes and cardiovascular disease in underserved populations, exacerbated by limited access to nutritious food. In this CME Outfitters Snack, expert faculty will share evidence-based strategies to integrate SDOH screening into healthcare practices, collaborate with community partners, and address the social needs of Black patients. Participants will learn how to use community resources to help patients manage chronic diseases and improve their overall health by addressing key drivers like food insecurity.
