Os odontoideum is a rare cervical anomaly stemming from a developmental failure in the odontoid process. This condition has an uncertain estimated prevalence, primarily due to its typically asymptomatic nature and the lack of extensive screening studies. Os odontoideum is radiologically characterized by an oval or round-shaped ossicle with smooth circumferential cortical margins, signifying a hypoplastic odontoid process that lacks continuity with the C2 vertebral body, posing challenges in diagnosis, especially when the ossicle is small and cephalic. Although this condition often remains asymptomatic, it can lead to severe cervical instability, emphasizing the critical importance of prompt diagnosis to mitigate the risk of spinal cord compression. One of the main risks of this anatomical entity is the association of anterior atlantoaxial subluxation, which, if left untreated, can lead to cervical spinal stenosis with resultant cervical myelopathy due to vascular compromise, bony compression, or stretching of the spinal cord. The etiology of this condition is disputed and can be either traumatic or congenital. Os odontoideum is usually found incidentally in the pediatric population or diagnosed in symptomatic patients at adult age.
