Pains in the Neck: Treating Globus Pharyngeus (AMW2023)

PROGRAM DESCRIPTION: Increasingly hypoglossal nerve stimulation is used to treat sleep apnea, while successful for a subset of patients, many patients require additional or ancillary treatment. When to apply palatal surgery in surgical treatment algorithms has become an even more complex challenge. Palatal surgery and snoring procedure may be indicated as isolated procedures or as part of multimodality treatment. This session will address the two author s approach to minimally invasive snoring and mild apnea as well as techniques of palate reconstruction. Snoring procedures will discuss not only techniques but a realistic review of outcomes. Reconstructive palatopharyngoplasty procedures will address techniques of anterior palatoplasty and expansion sphincterplasty. Discussion will include complications and side effects of procedures and post operative management including selection of patients for observation and pain management.OUTCOME OBJECTIVE 1: Describe key anatomic landmarks and structures critical for performing reconstructive palatopharyngoplasty including the structure and importance of PPP muscle and lateral palatal fatOUTCOME OBJECTIVE 2: Understand application of several surgical approaches to PPP including the modified Australian UPPP, expansion pharyngoplasty, dorsal palatal flap closure, and barbed suturesOUTCOME OBJECTIVE 3: Compare office based surgical techniques for snoringBACKGROUND STATEMENT: Palatal surgery techniques are evolving. This symposium will update the current status of palate surgery in the era of neuromodulation.
