Pediatric Chronic Nasal Obstruction: Beyond the Adenoidectomy (2022 AMW)

Chronic nasal obstruction in the younger pediatric population is one of the most common chief complaints encountered by otolaryngologists the world-over. From rhinitis of infancy to chronically snotty noses in 5 year olds status post adenoidectomy, pediatric nasal obstruction is a challenging, often-times frustrating, disease process to navigate. It can baffle even the most experienced generalists and pediatric subspecialists. Not to mention the profoundly negative effect it has on the quality of life of our youngest patients and their families. The goal of this panel is to explore the stepwise management strategies, both medical and surgical, of experts in the field of pediatric otolaryngology and pediatric rhinology who care for these patients daily. Through case-based discussions, we will explore the use of ophthalmic steroid drops in treating rhinitis of infancy, the use of daily saline and suctioning in the chronically obstructed daycare child, the off-label use of intranasal steroids in children under the age 5, initiation and duration of antibiotic therapy, the use of immunotherapy, and the role of surgery and/or devices in the treatment of this disease process. We will also discuss the timing and role of allergy testing, immunologic work up, sweat chloride testing, nasal nitric oxide testing, and ciliary biopsy, amongst other testing strategies, in the care of these patients.
