Pediatric Respiratory Emergencies

The pediatric patient often presents a significant diagnostic challenge. Respiratory illness is a common pediatric presentation to Emergency Departments, clinics, and private offices; the chief complaints of “cough,” “congestion,” “noisy breathing,” and “can’t breathe” are familiar to all practitioners who care for infants and children. Problems that can affect the pediatric respiratory system range from mild upper respiratory infections to life-threatening infections of the middle or lower airway. Shock syndromes and respiratory problems are the major causes of pediatric arrests. Therefore, early recognition of potential respiratory failure in pediatrics is crucial.

In this activity, we review clinical information and discuss common medical errors and allegations related to frequent as well as infrequent respiratory maladies. We examine a number of cases to highlight some of the reasons why these diagnoses are often missed, delayed, or inappropriately managed. After completing this activity, practitioners should be able to: recognize the signs and symptoms of potential respiratory failure and serious bacterial infection in pediatric patients who present with respiratory symptoms in order to make an early diagnosis and initiate appropriate treatment; compare and contrast the signs and symptoms of upper and lower airway infections in order to distinguish between them in the evaluation of pediatric patients who present with respiratory symptoms; and apply knowledge of the factors that contribute to delayed or missed diagnoses to the evaluation of pediatric patients who present with respiratory symptoms in order to improve patient safety and decrease the risk of exposure to medical malpractice.
