Phenobarbit-All-In: Evaluating Phenobarbital Use in Alcohol Withdrawal

Phenobarbital has shown potential in the management of alcohol withdrawal syndrome (AWS), particularly as an adjunct or monotherapy alternative to benzodiazepines. However, there is a lack of standardized protocols for its use in the inpatient setting, making it difficult for clinicians to confidently integrate phenobarbital into inpatient care where benzodiazepines may be ineffective or fail. This presentation discusses the pathophysiology of alcohol withdrawal, and the pharmacology of benzodiazepines and phenobarbital; then, the presentation evaluates the available primary literature on phenobarbital use as an alternative first-line agent, its efficacy and safety of, and highlights key recommendations based on the studies available and the current Ascension Texas protocol.
