The New Mexico PMS ECHO Program is a collaboration with Presbyterian Medical Services (PMS). Through iterative guided practice, providers gain expertise in administering specialized medical care in primary care settings. Unlike traditional direct service or specialty consultation telehealth models, the ECHO model works on the principle of workforce multiplication, expanding access to best practice specialty healthcare by increasing primary care team capacity to care for diabetes mellitus and multiple chronic conditions (DMMC) patients closer to their homes. The goals include increased patient activation and satisfaction with care for DMMC patients empaneled to primary care providers (PCPs) in the ECHO arm relative to the usual care arm and increased adoption of clinical best practices among PCPs in the ECHO arm relative to the usual care arm, including appropriate screenings and prescriptions.
