With a historical perspective on the evolution of polypharmacy, this session emphasizes its contemporary definition based on current data, focusing on evidence-based practice and the importance of reducing inappropriate polypharmacy. As the United States grapples with one of the highest medication rates worldwide, healthcare providers face the challenge of optimizing medication use to enhance patient outcomes. This activity aims to equip participants with practical strategies to navigate polypharmacy, emphasizing the goal of improving medication management skills and ultimately enhancing the quality of care provided to those at risk of adverse consequences of polypharmacy.
